Exam Profile Form

All questions must be numbered. Any question that is not a multiple choice question is considered and essay question.

Faculty members are responsible for proofreading their exams and ensuring that a complete exam is submitted to the Faculty Coordinator via e-mail and/or hand-delivery. The Faculty Coordinator will copy the document submitted and ensure that all pages submitted are copied.
